How to apply to DASMHP

Four simple steps — download the form, fill it out, gather your documents, then submit everything at the school office.

Fill the form carefully

Complete every section — personal details, address, parents/guardian information, and academic qualifications. Use clear handwriting.

Gather all required documents

Once the form is filled, prepare the following to submit together with the form:
- Application letter (handwritten, addressed to the Principal)
- Photocopy of "A" or "O" Level certificates
- Photocopy of National Identity Card (NIC)
- Photocopy of Birth Certificate
- 4 Passport-size photos (4×4)
- The completed admission form

Submit at the school head office

Hand-deliver the completed form and all supporting documents together to the admissions office:

All documents must be clear, legible photocopies. Incomplete files will not be processed.

Application fee: Free. Registration fee: 10,000 FCFA paid at the school secretariat when you submit your application documents. Uniform fee: 20,000 FCFA paid at the school secretariat. Tuition fees: Paid at ECOBANK.
